Pediatric Physician or Nurse Practitioner (Part-time, On-Call)
To provide professional primary care medical services for patients in accordance with clinic procedures and protocols and sound medical practices. Take medical histories, conducting physical examinations, ordering lab tests and prescribing treatments and medications. Perform minor surgical procedures, adapt treatment programs as indicated and refer patients for consultation and special services. Discuss medical progress with patients and family. Maintain up-to-date patient case records and prepare reports as indicated. Required: Current Medical License, DEA, CPR. Preferred: Experience in public or community health, bilingual. To coordinate primary medical care, patient education, and treatment adherence for HIV+ and all State Street patients, and to interface with other entities including specialty care providers, pharmacies and other Bay Area clinics and hospitals to assure access to care. Provide medical services in conjunction with the HIV Provider, including patient assessment and routine lab procedures. Coordinate specialty care and other needed services for patients and assure follow-up with medical Care Plan. Participate in bi-weekly case conferences with HIV Care Team and work in collaboration with Case Managers and Client Services Advocates to maintain HIV clients in care, establish maximum use of client entitlement and insurance programs, and coordinate the flow of services for each patient. Represent agency at HIV ACCESS network meetings and at other meetings required by funders of HIV medical program. Maintain documentation for case reporting as required. Requires: Current RN or NP license, Experience performing blood draws, Prior work related experience with HIV+ populations, active substance users, homeless individuals, and other vulnerable groups preferred. Clinical Programs Manager
Under limited supervision, the Clinical Programs Manager is primarily responsible, accountable and provides oversight for the management, budget, coordination and direction and other non medical administrative functions for a variety of ancillary clinical programs including, but limited to, Alameda County Excellence (ACE), Homeless, HOPE, Senior Center, and other various grant funded programs. The incumbent will need to work collaboratively with the medial providers regarding chronic illness patient health issues and with the Marketing and Outreach Department to develop targeted outreach efforts for specific populations in the area of chronic disease (i.e., asthma, congestive heart failure, diabetes, obesity, hypertension). This individual will identify educational needs and develop self-management educational tools through collaboration with health care providers, various staff and outside vendors to provide patients with an ongoing chronic disease case management program. Develops and implements operation audits and leads program team in implementing evidence-based practices that are sensitive to limited resources and reports on the progress of the programs. Requires: Minimum of two years of related experience including the oversight of a medical program, preferably with chronic illnesses. Experience in a leadership role indicated by successfully forming a team that is highly productive and demonstrates collaborative relationships with multi-disciplinary teams. One year of supervisory experience or as lead person overseeing the implementation of a program of significant size and scope. Preferred: BA degree in Health Sciences Administration, MPH or other related program. Community health center work related experience. LVN Highly desirable; RN preferred. Grant and budget experience.
