Outreach Services

Tri-City Health Center (TCHC) provides a variety of outreach services which interact with schools, businesses, as well as other community organizations in an effort to develop locally responsive strategies that can effectively meet the needs of our community. These strategies address the costly effects of devastating health problems, including substance abuse, domestic violence, infant mortality, homelessness, HIV/AIDS, mental health problems, and other infectious diseases. We strive to create strong partnerships with local community leaders to join together in support of better community health.

Tri-City Health Center's Outreach Services Include:

Health Education

We provide a variety of health education programs including nutritional counseling, contraceptive and reproductive counseling, chronic disease management, as well as parenting classes that inform pregnant women as well as prospective and new parents about the importance of pre-natal care, child safety (at home, in the streets, and on the highway), non-violent child discipline, early childhood development, literacy, and cultural diversity issues involved in child rearing.

Risk-Group Specific Outreach

Because we serve high-risk and vulnerable populations, our outreach services allow us to design innovative programs that ensure that patients have access not only to medical treatment but to a continuum of coordinated care and vital support services that can lead to positive health outcomes and healthier behaviors and lifestyles. These services include our Men Together Project (Link), TransVision, outreach to at-risk teens, a Female Empowerment Project (FEM), a Needle Exchange, as well as several programs targeting the at-risk teens in our community.

The clinic is open from 8am-8:30pm Monday through Thursday and 8am-5pm Friday and Saturday.
