Alameda County Excellence (ACE) Program is a new chronic disease management program for Alameda County adult residents. Patients who meet ACE eligibility guidelines and have diabetes, hypertension, congestive heart failure or asthma may qualify for this program. Benefits include:

• Case manager who will follow your care and answer your questions
• Access to specially tailored education materials, resources and classes
• Appointment reminder for primary care physician (PCP) and specialty visits
