Diabetes Program
We have 6 doctors, 5 nurse practitioners, a diabetes nurse case manager, a registered dietician and a retinopathy (eye) specialist dedicated to helping our diabetic patients stay healthy.
Once diagnosed with diabetes you will be referred to see our diabetes case manager, and to our diabetes education class. During the diabetes education class you will learn how to manage your disease. After the class you will be seen every 3-months by a doctor or a nurse, and by our diabetes case manager.
During the education classes you will learn about:
- The causes of diabetes
- Complications of diabetes
- How to check your blood sugar and what the results mean
- The benefits of exercise
- How your eating habits affect how you feel
- How to read labels at the grocery store
Through our diabetes program you might also be eligible to receive:
- Blood sugar test machine, testing strips, and lancets
- Medications
- Glucose tablets
- Sharps container
