A TCHC community case worker provides outreach to the homeless living in our community keeping them informed about the medical and social services we can provide for their health care needs.
Individuals needing medical attention have access to a physician, nurse practitioner, or physicians assistant at our three main clinics or six satellite sites.
In addition to providing medical services, TCHC also provides:
- lab work
- prescription drugs
- radiology
- case management
- financial benefits counseling
- access to housing assistance and support services
