Primary Care Team Approach
TCHC’s care team coordinates your primary health care needs. Our clinical staff includes:
- family practice and internal medical physicians
- family practice nurse practitioners
- physician assistants-certified
- case managers
- mental health specialists
- nurses
- nutritionists
- medical assistants
- health educators
We assign each patient to a Primary Care Provider, or PCP. We try to schedule all of your medical appointments with your PCP.
- Your PCP may be a doctor, nurse practitioner or physician assistant.
- Your PCP, case manager or health educator will help you with referrals to specialists or other providers, if needed.
