Q: Where is the Teen Center located?
A: Teen City Clinic is located at 39500 Liberty Street, Fremont, CA 94536. Entering in through Liberty Street, you can follow the parking lot to the back, making an automatic right turn. Teen City Clinic entrance will be on your right hand side.
Q: What kind of paperwork do I need to fill out?
A: Each new patient must fill out a registration form regarding contact information. REMEMBER: we need an address but we will NOT send anything to that address unless it's an emergency and we cannot reach you through your phone number, or emergency contact’s phone number. You also fill out a men's or women's health history questionnaire, as well as an application for the program that pays for the visit.
Q: What happens during my appointment?
A: A first time patient comes the day of the appointment, fills out paperwork, sees a Teen Clinic Counselor, then meets with the provider. During your visit with the counselor, we will talk about the main reason of your visit, education on Birth Control Methods/Use, an STD Risk Assessment, and general information about your sexual health and Teen Clinic.
Q: Is the Teen Clinic only for teens?
A: Teen City Clinic is a youth-friendly clinic that has a private entrance, trained youth counselors, private waiting area, a blocked telephone number, and other teen-sensitive protocols in place to assure confidentiality for youth ages 12 to 24.
Q: Do I need a parent's consent to get services?
A: A parent's consent/approval is not required to get services at Teen City Clinic, and we will do all that we can to ensure confidentiality and privacy.
Other youth family planning activities include street and venue-based outreach and presentations at mainstream and continuation high schools, and targeted outreach, workshops and community events aimed at increasing responsibility in preventing teen pregnancy.
